It's very easy to open a Program account. To take care of the process for your client:

1. Complete the Contribution Agreement Form for Financial Professionals and Donation of Assets Form for Financial Professionals. For your convenience, you can complete the forms online before printing them out or download them and complete by hand. Have your clients sign the forms prior to sending them to the Program.

2. If the client is making a contribution

• By check — include the check for the irrevocable contribution of $10,000 or more with the above forms.
• By a transfer of stocks in kind to the Program's account — complete or have your client complete and sign the Delivery Instructions form authorizing the financial institution holding the assets to make the transfer. Send the original form by mail or fax to the broker or financial institution holding the stocks.
• By a transfer of mutual funds — please include with the Program's forms the client's most recent account statement from the financial institution holding the mutual funds.
  
  

3. Return the completed Contribution Agreement and Donation of Assets Form as indicated below. Include a check if the contribution is in cash. Include a copy of the signed Delivery Instructions form if the gift is stock being transferred from a financial institution other than T. Rowe Price. Include the donor's most recent account statement if the gift is mutual funds held at a financial institution other than T. Rowe Price:
